Please print out the form, fill in and submit it in person to the service desk together with the other required documents.

Consent Form for Registration of Underage Users [de] 11 K
Parents or legal guardians have to use this form to declare their consent for the registration of an underage user. This form must be submitted on registration, together with the national ID card or a parent or legal guardian.

Loan Request for Conventional Interlibrary Loans [de] 51 K
Please fill out the form on a computer and submit the printed form to the service desk of the Central University Library.

Delivery Note for Academic Staff Members [de] 19 K
Academic staff members can request invoices for the fees due at the University Library. For this, the staff members have to fill out a delivery note. The fees can also be paid at the service desk.
